It is a point based system that with each 45 minutes in game time you can get a loyalty point.
In order to see how many points you have you must access the loyalty menu.

Loyalty can be accessed in game by using the command .Loyalty
To refresh the time you have, you must restart your game.

For every 45 minutes you can redeem one loyalty point by clicking the Redeem Points option.

Loyalty Points are used in the Loyalty Point Shop

When buying from the Loyalty Points Shop your points will be converted to Spent Points that can then be used in the Loyalty Milestone Rewards.

The Loyalty Milestone Rewards can only be used with Spent Points. And every time has to be purchased in order from the First one to the Last one.
